Editor’s Note: This month’s selection focuses on a practical question at the heart of evidence-based medicine: when does an intervention improve a clinically meaningful outcome, and when does it mainly change an intermediate measure or a care process? Across acute cardiovascular care, perioperative medicine, stroke, oncology, tuberculosis and paediatric haematology, the studies below reinforce the value of distinguishing biological or procedural success from patient-centred benefit. Several interventions appear useful when they solve a defined clinical problem, while others caution against intensification simply because a treatment is feasible, plausible or capable of improving a surrogate endpoint. The common lesson is to match implementation to the outcome actually demonstrated, while remaining attentive to generalisability, treatment burden and the need for confirmation in broader populations.

LDL Cholesterol Lowering With Evolocumab Before Percutaneous Coronary Intervention for Acute Myocardial Infarction: The AMUNDSEN Randomized Clinical Trial.

Summary
After acute myocardial infarction, rapid achievement of recommended low-density lipoprotein cholesterol targets is desirable, but usual stepwise escalation can leave patients above target for prolonged periods. The AMUNDSEN trial concluded that initiating evolocumab in the catheterisation laboratory, alongside intensive oral lipid-lowering treatment, produced rapid and sustained LDL-cholesterol reduction and substantially improved attainment of guideline targets. However, this earlier and more intensive strategy did not translate into detectable clinical benefit during the first year. The study therefore supports immediate PCSK9 inhibition as an effective way to improve lipid control, but not as evidence of an acute outcome benefit beyond LDL lowering. Longer follow-up is needed before routine first-line use can be justified on clinical-event grounds alone.
FR Après un infarctus aigu du myocarde, il est souhaitable d’atteindre rapidement les objectifs recommandés de cholestérol LDL, mais l’intensification thérapeutique progressive habituelle peut laisser les patients au-dessus de la cible pendant une période prolongée. L’essai AMUNDSEN conclut que l’instauration de l’évolocumab en salle de cathétérisme, associée à un traitement hypolipémiant oral intensif, entraîne une réduction rapide et durable du cholestérol LDL et améliore nettement l’atteinte des objectifs recommandés. Cette stratégie plus précoce et plus intensive ne s’est toutefois pas traduite par un bénéfice clinique détectable au cours de la première année. L’étude soutient donc l’utilisation précoce d’un inhibiteur de PCSK9 pour améliorer le contrôle lipidique, mais ne démontre pas de bénéfice clinique aigu au-delà de la baisse du LDL. Un suivi plus long est nécessaire avant de justifier une utilisation systématique en première intention sur la seule base des événements cliniques.

El ensayo AMUNDSEN concluyó que iniciar evolocumab en la sala de cateterismo, junto con un tratamiento hipolipemiante oral intensivo, produjo una reducción rápida y sostenida del colesterol LDL y mejoró claramente la consecución de los objetivos recomendados. Sin embargo, esta estrategia más precoz e intensiva no se tradujo en un beneficio clínico detectable durante el primer año. Por tanto, el estudio respalda la inhibición inmediata de PCSK9 como una forma eficaz de mejorar el control lipídico, pero no como prueba de un beneficio clínico agudo más allá de la reducción del LDL. Summary
Preoperative anaemia is common in cardiac surgery and is associated with greater transfusion exposure and more difficult recovery, yet evidence that correction improves patient-centred outcomes has been limited. The ITACS trial concluded that preoperative intravenous iron increased the time patients were alive and at home during early recovery and reduced red-cell transfusion. Importantly, benefit was observed even though enrolment did not require confirmed absolute iron deficiency, suggesting that a pragmatic anaemia-based approach may be clinically useful. The findings strengthen the case for incorporating timely intravenous iron into preoperative blood-management pathways for elective cardiac surgery, while recognising that local protocols should still address the cause of anaemia and the interval available before surgery.
FR L’anémie préopératoire est fréquente en chirurgie cardiaque et est associée à une exposition transfusionnelle accrue ainsi qu’à une récupération plus difficile, alors que les preuves montrant qu’une correction améliore les résultats centrés sur le patient restaient limitées. L’essai ITACS conclut que l’administration préopératoire de fer intraveineux augmente le temps passé en vie et à domicile au début de la convalescence et réduit le recours aux transfusions de globules rouges. Fait important, un bénéfice a été observé sans exiger la confirmation d’une carence martiale absolue, ce qui suggère qu’une approche pragmatique fondée sur l’anémie peut être cliniquement utile. El ensayo ITACS concluyó que el hierro intravenoso preoperatorio aumentó el tiempo que los pacientes permanecieron vivos y en su domicilio durante la recuperación inicial y redujo la necesidad de transfusión de hematíes. Es importante que el beneficio se observó sin exigir la confirmación de una deficiencia absoluta de hierro, lo que sugiere que un enfoque pragmático basado en la anemia puede ser clínicamente útil. Summary
Head position after thrombectomy has a plausible effect on cerebral haemodynamics, but the optimal routine strategy after successful reperfusion has remained uncertain. The HeadSOAR trial concluded that maintaining an elevated head position after successful endovascular thrombectomy did not improve functional outcomes compared with lying flat. The result argues against treating a single post-procedural position as universally beneficial once reperfusion has been achieved. Instead, positioning can be individualised according to airway protection, aspiration risk, intracranial pressure, cardiopulmonary status and the clinical context. The study is a useful reminder that physiological rationale does not necessarily translate into better functional recovery and that post-thrombectomy care should remain responsive to the needs of the individual patient.
FR La position de la tête après thrombectomie peut plausiblement modifier l’hémodynamique cérébrale, mais la stratégie systématique optimale après une reperfusion réussie restait incertaine. L’essai HeadSOAR conclut que le maintien de la tête en position surélevée après une thrombectomie endovasculaire réussie n’améliore pas le résultat fonctionnel par rapport au décubitus à plat. Ce résultat va à l’encontre de l’idée qu’une position post-procédurale unique serait universellement bénéfique une fois la reperfusion obtenue. Le positionnement peut plutôt être individualisé selon la protection des voies aériennes, le risque d’inhalation, la pression intracrânienne, l’état cardiopulmonaire et le contexte clinique. L’étude rappelle utilement qu’une justification physiologique ne se traduit pas nécessairement par une meilleure récupération fonctionnelle et que les soins après thrombectomie doivent rester adaptés aux besoins de chaque patient.

El ensayo HeadSOAR concluyó que mantener la cabeza elevada después de una trombectomía endovascular satisfactoria no mejoró los desenlaces funcionales en comparación con la posición plana. El resultado desaconseja considerar una única posición posprocedimiento como universalmente beneficiosa una vez conseguida la reperfusión. En su lugar, la posición puede individualizarse según la protección de la vía aérea, el riesgo de aspiración, la presión intracraneal, el estado cardiopulmonar y el contexto clínico. Addition of High-Dose Vitamin D3 to Standard Treatment in Patients With Metastatic Colorectal Cancer: The SOLARIS Randomized Clinical Trial (Alliance A021703).

Summary
High-dose vitamin D has attracted interest as a low-cost adjunct to systemic treatment for metastatic colorectal cancer, particularly after an earlier trial suggested a possible benefit. The phase 3 SOLARIS trial concluded that adding high-dose vitamin D3 to standard chemotherapy plus bevacizumab did not improve progression-free survival compared with standard-dose supplementation. The study therefore does not support routine high-dose vitamin D as an anticancer strategy in this setting. Its wider significance is methodological as well as clinical: encouraging signals from smaller studies require confirmation before supplementation is incorporated into oncology pathways. Standard vitamin D use may still be appropriate for conventional nutritional or bone-health indications, but escalation to high doses should not be presented as a disease-modifying cancer treatment on the basis of current evidence.

Evaluation of patient-centred digital adherence technology for tuberculosis treatment outcomes: pragmatic randomised controlled trial.

Summary
Adherence to prolonged tuberculosis treatment is difficult to sustain, and digital tools are often proposed as a scalable solution without clear evidence that they improve treatment completion. This pragmatic trial concluded that a patient-centred digital support system modestly improved treatment outcomes in people receiving self-administered therapy for drug-susceptible tuberculosis. The intervention combined daily adherence reporting with bidirectional communication, educational material and verification, making it more than a simple reminder application. The study supports digital adherence technology as an adjunct to tuberculosis services when it strengthens contact between patients and treatment supporters. Its effectiveness is likely to depend on workflow integration, sustained engagement and local implementation capacity, so replication should focus on the whole care model rather than the software alone.

Daily Zinc Supplementation for Infection Prevention in Children With Sickle Cell Anemia: The ZIPS-2 Randomized Clinical Trial.

Summary
Young children with sickle cell anaemia remain highly vulnerable to infection despite established preventive measures, and zinc deficiency may contribute to impaired immune defence. The ZIPS-2 trial concluded that daily zinc supplementation reduced all-cause infections in children younger than five years with sickle cell anaemia in Uganda, without treatment-limiting adverse effects. The finding is clinically attractive because the intervention is inexpensive, simple and potentially scalable in settings where infection burden is high. However, this was a single-site study in a specific age group, and the investigators appropriately call for multisite confirmation and evaluation in older children. The result should therefore be viewed as a promising adjunct to standard sickle-cell preventive care rather than an immediate universal supplementation policy.
