Editor’s note: This month’s selection asks how lung-health services can convert plausible interventions into reliable patient benefit. Across severe asthma, pulmonary fibrosis, perioperative respiratory care, viral outbreaks in long-term care and small-cell lung cancer, the studies below show that progress depends as much on defining limits as on expanding treatment. Some findings support a more targeted use of established or emerging therapies; others caution that physiological improvement, technical availability or treatment intensification does not automatically improve daily function, survival or safety. Together, they reinforce a disciplined approach to respiratory evidence: match the intervention to the population, reproduce the tested pathway faithfully and judge success by outcomes that matter to patients.

Summary
This trial is clinically important because long-term oral corticosteroid dependence remains a major source of cumulative harm in severe asthma. The central conclusion is that tezepelumab enabled meaningful reduction of maintenance oral corticosteroid treatment while preserving asthma control. The study therefore strengthens the role of upstream type 2–independent biologic therapy as a steroid-sparing strategy in carefully selected adults whose disease remains dependent on systemic corticosteroids. Clinically, the result supports structured tapering rather than abrupt withdrawal, with continued assessment of exacerbations, symptoms, adrenal function and the patient’s broader inflammatory phenotype.

Ambulatory oxygen for treatment of exertional hypoxaemia in pulmonary fibrosis (PFOX): a multicentre, randomised, sham-controlled trial.

Summary 
This trial is clinically useful because ambulatory oxygen is frequently prescribed to people with fibrotic interstitial lung disease who desaturate during exertion, despite uncertainty about its effect on everyday activity. The central conclusion is that oxygen delivered through a portable concentrator did not improve physical activity, breathlessness, fatigue or health-related quality of life compared with ambulatory air. The result does not negate oxygen for established resting hypoxaemia or for selected symptom-responsive patients, but it argues against assuming that correction of exertional oxygen saturation alone will improve daily functioning. Clinically, prescription should be individualised and linked to a demonstrable goal that matters to the patient.

La conclusión central es que el oxígeno administrado mediante un concentrador portátil no mejoró la actividad física, la disnea, la fatiga ni la calidad de vida relacionada con la salud frente al aire ambulatorio. El resultado no invalida el oxígeno para la hipoxemia en reposo establecida ni para pacientes seleccionados con respuesta sintomática, pero desaconseja asumir que corregir la saturación durante el esfuerzo mejorará por sí solo la función diaria. Summary 
This trial is important because residual neuromuscular blockade is a preventable contributor to postoperative respiratory morbidity. The central conclusion is that sugammadex reduced the composite risk of postoperative pulmonary complications or death compared with neostigmine in adults undergoing abdominal or thoracic surgery, although the absolute benefit was small and was driven largely by atelectasis of uncertain longer-term significance. Clinically, sugammadex can be considered a first-line reversal agent for aminosteroid neuromuscular blockade, but drug choice should not replace quantitative monitoring, appropriate dosing, careful extubation criteria and broader perioperative lung-protection measures.

Respiratory Outbreak Mitigation With Point-of-Care Testing in Long-Term Care: A Randomized Clinical Trial.

Summary 
This cluster-randomised trial is clinically relevant because residents of long-term care facilities are especially vulnerable to influenza, respiratory syncytial virus and SARS-CoV-2, while delays in laboratory testing can slow treatment and infection-control decisions. The central conclusion is that on-site multiplex polymerase chain reaction testing did not reduce the number or size of outbreaks, but it improved case detection, accelerated antiviral treatment for influenza and reduced emergency-department transfers. The study therefore shifts the implementation question away from outbreak prevention alone: rapid diagnostics may be most valuable when embedded in a pathway that converts results promptly into isolation, treatment and escalation decisions.

Consolidative Thoracic Radiotherapy With Atezolizumab Maintenance in Extensive-Stage Small Cell Lung Cancer: The Phase 2 TREASURE Randomized Clinical Trial (AIO-TRK-0320).

Summary 
This trial is valuable because consolidative thoracic radiotherapy has a plausible local-control rationale after response to first-line chemoimmunotherapy in extensive-stage small-cell lung cancer. The central conclusion is that adding thoracic radiotherapy to atezolizumab maintenance increased serious toxicity, particularly infections, without improving survival, leading to early termination of recruitment. The findings argue against routine use in unselected patients and raise concern that radiation-related lymphocyte depletion may undermine the safety of combined-modality treatment. Clinically, any future use should be confined to carefully selected patients or trials with explicit safety monitoring, attention to treatment sequencing and strategies to limit radiation exposure to highly perfused organs.
