Tozorakimab to Prevent COPD Exacerbations.

Summary
The replicate OBERON and TITANIA trials found that adding tozorakimab, an antibody targeting interleukin-33, to maintenance inhaled therapy reduced moderate or severe COPD exacerbations. Benefit was demonstrated in the primary population of former smokers and in the overall population, which also included current smokers. Enrolment was not restricted by blood eosinophil count. The findings support a new therapeutic approach for exacerbation-prone COPD, while their interpretation should remain focused on prevention of exacerbations rather than an assumed survival benefit.

Azithromycin for Preschoolers with Wheezing in the Emergency Department.

Summary
The AZ-SWED trial found that azithromycin did not reduce wheezing-related symptom severity compared with placebo in preschool children presenting to emergency departments with moderate-to-severe acute wheeze. The finding argues against routinely adding an antibiotic to treat the wheezing episode itself. This conclusion concerns acute wheeze in the studied emergency setting and does not remove the need to treat a separately diagnosed bacterial infection. Clinically, treatment decisions should follow the respiratory presentation and any specific infectious indication.

Se refiere al contexto agudo estudiado y no elimina la necesidad de tratar una infección bacteriana diagnosticada por separado. Las decisiones deben responder a la presentación respiratoria y a cualquier indicación infecciosa específica.
Oxygen saturation thresholds in children with acute respiratory distress (OxyKids): a multicentre, open, parallel-group, randomised clinical trial.

Summary
OxyKids found that a lower oxygen saturation threshold reduced oxygen treatment and hospital stay in children admitted to general paediatric wards with bronchiolitis, lower respiratory tract infection or acute viral-induced wheeze. There was no evidence of short-term harm in the assessed outcomes. The result suggests that conventional thresholds can prolong treatment unnecessarily in selected children. It supports reassessing ward oxygen protocols within the studied clinical context, without assuming applicability to critically ill children or establishing long-term safety.

First-Line Sunvozertinib in NSCLC with EGFR Exon 20 Insertion Mutations.

Summary
WU-KONG28 found that first-line sunvozertinib prolonged progression-free survival compared with carboplatin–pemetrexed in advanced non-squamous non-small-cell lung cancer with EGFR exon 20 insertions. The trial supports a targeted treatment option for a molecular subgroup historically difficult to treat. Severe adverse events were more frequent with sunvozertinib, and overall-survival data remained immature. The practical implication is to pair accurate molecular diagnosis with careful toxicity management and distinguish improved disease control from an established survival advantage.

Health Communication and Stepped Reminders Interventions for Lung Cancer Screening: A Randomized Clinical Trial.

This pragmatic trial found that appropriately timed reminders to primary care clinicians to order scans, coupled with patient outreach to arrange them, improved adherence to annual lung cancer screening after a normal initial scan. Educational print and video communication did not provide the same benefit. The effective intervention combined a screening coordinator, an electronic registry and reminders addressing practical steps. The study supports organised follow-up pathways; it demonstrates improved screening completion rather than a direct reduction in lung cancer mortality.
