Editor's Note: This month’s selection examines how psychiatric care can address persistent symptoms, physical health and the circumstances that influence treatment decisions. The six publications cover resistant bipolar depression, negative symptoms of schizophrenia, antipsychotic-associated weight gain, dietary intervention, depression during pregnancy and adolescent gaming problems. They offer different kinds of evidence: controlled treatment trials, comparative synthesis, a feasibility pilot and an evidence-informed clinical review. Their practical value depends on preserving those distinctions. Short-term benefit does not settle maintenance treatment, weight reduction does not prove cardiovascular protection, and a benefit confined to higher-risk adolescents should not be presented as universal prevention.
Serial Ketamine Infusions for Treatment-Resistant Bipolar Depression: A Randomized Clinical Trial.

This randomised trial found that repeated intravenous ketamine infusions, added to stable mood-stabilising or antipsychotic treatment, improved treatment-resistant bipolar depression more than midazolam. Treatment was well tolerated during the short controlled period, without observed mania or hypomania. These findings extend the evidence for ketamine to a population distinct from unipolar depression. They do not establish a maintenance strategy or exclude uncommon mood-switching risks. The clinical implication is a promising specialist treatment option requiring continued evaluation of durability and longer-term safety.

A Digital Therapeutic Intervention for Negative Symptoms of Schizophrenia: A Randomized Clinical Trial.

The CONVOKE trial found that the smartphone-based therapeutic CT-155 improved motivation and pleasure-related negative symptoms compared with a digital control when added to antipsychotic treatment. It was well tolerated within the study. The result supports targeting persistent negative symptoms through structured psychosocial content delivered digitally. It does not show that the intervention replaces medication or improves every negative-symptom domain. Participants needed stable housing, smartphone access and stable treatment; effectiveness and access in less supported settings therefore require further evaluation.

Pharmacological Interventions for Weight Reduction in Patients With Schizophrenia Treated With Antipsychotics: A Systematic Review and Network Meta-Analysis.

This network meta-analysis found substantial differences between medicines used to address weight gain in antipsychotic-treated schizophrenia spectrum disorders. Semaglutide, liraglutide, topiramate, metformin and exenatide had the strongest evidence for weight reduction, with moderate rather than high certainty. The findings support active management of metabolic adverse effects alongside psychiatric treatment. Comparative rankings do not establish one medicine as the best choice for every patient, and weight reduction should not be equated with proven cardiovascular benefit. Treatment selection must consider individual suitability, tolerability and access.

Reducing Ultraprocessed Foods From the Diets of Patients With Depression: A Pilot Crossover Trial.

This small, open-label crossover pilot found that reducing ultraprocessed food consumption was feasible and acceptable for people with depression and high baseline consumption. Depressive and anxiety symptoms were lower during the reduced-consumption period, but these were exploratory outcomes rather than the study’s primary purpose. The result supports larger controlled dietary trials, not an established antidepressant claim. Self-reported intake, lack of blinding and changes in other dietary features limit interpretation. Nutritional support can be considered within broader care without replacing established depression treatment.

Depression and SSRI Treatment During Pregnancy-Prioritizing Maternal Mental Health.

This evidence synthesis argues that maternal depression must remain central when evaluating selective serotonin reuptake inhibitor treatment during pregnancy. Much of the safety literature is complicated by the effects of the underlying illness; separating these from medicine exposure gives a more reassuring interpretation of serious adverse outcomes. The authors support structured decisions covering untreated illness, treatment benefits and exposure risks. This is a special communication, not a new treatment trial or a claim of universal safety. Its practical contribution is balanced counselling and coordinated access to perinatal mental-health care.

Cognitive Behavioral Therapy vs Media Literacy for Prevention of Gaming Disorder and Unspecified Internet Use Disorder: A Randomized Clinical Trial.

The PROTECTconfirm trial compared school-based cognitive behavioural prevention with a parallel media-literacy programme. CBT reduced gaming and unspecified internet use disorder symptoms more effectively among adolescents already at higher risk, but superiority was not established in the full sample. The findings support indicated prevention for emerging difficulties rather than assuming that the same programme benefits every pupil equally. They concern symptom severity, not proof that all future diagnoses are prevented. Trained delivery and supervision also formed part of the tested intervention and matter when considering implementation.
